When interviewed the provider reported that she had knocked a preschool child's hands away from her face with the provider's forearm because the child was putting them into her mouth after touching her genitals. She acknowledged exclaiming, "Stop!". The Center Assistant Director reported observing the provider use a "loud tone" multiple times, on previous occasions.

The small cement square at the entrance to the preschool playground has a tripping hazard-a hole adjacent to it. Large double gate between alley and outdoor playground is latched but does not shut all the way. A provider attempted to shut it all the way. This brought the gap down to four inches from approximately five inches.

This cabinet was secured prior to our departure on 9.16 Windowsill edges, at infant height, in infant room, have sharp edges. Washer and dryer were accessible to children in an unlocked laundry room. A secondary freezer was accessible to children in the unlocked kitchen. The kitchen doorknob was replaced on 09.17. Sinks in Pre-K and Transitional Kindergarten, Developmental Preschool have visible rust around drain and faucet (Pre-school)

The provider has not requested, or is uncertain if she requested, a fire inspection from her local fire authority. The stove in the kitchen was accessible to children because the kitchen door was unlocked. The Director reports a new doorknob which is locking has been installed. The electrical motor in the fan above the stove is very dusty and dirty.

Freezer in freezer/refrigerator combo closest to kitchen door is missing thermometer in freezer. Refrigerator in this combo unit tested at 48 degrees. The other freezer/refrigerator combo next to this one did not have a thermometer that I could locate in the refrigerator portion of the unit. A number of food items, waffles and fishsticks in freezer, in the freezer and a bag of crackers on the counter, are not stored in their original box. Because of this, there is no expiration date on these items. The bag of crackers was shut loosely and it was not airtight. This licensor located three spices that had expired more than one year ago. Cornmeal was expired. A tub of sunflower butter stored on a shelf did not have an expiration date. There was also visible sun butter smeared on the outside of the container.

Spray starch, spray adhesive, paint in five gallon buckets, and shaving cream were observed accessible to children in unlocked storage closets. Except one, every storage area and room was unlocked on the date of inspection.

A provider was observed leaving eight children unsupervised, both auditorily and visually, when she went around the corner of the building to request that two other children return to the other side of the playground. She was out of auditory and visual supervision range for approximately three minutes. Providers were observed standing in doorways while children walked to bathrooms to use them independently. Due to the distance, the children were out of auditory range while in the bathroom, although the doors were open. Portions of the child's body could be seen moving in the bathroom. However, the providers did not notice that neither child flushed the toilet. One toilet seat had urine on it following the child's time using it.

Shashi was working in the infant classroom and in ratio today without this training. Acting Director was unable to locate verification of a Food Handler's Permit for Uma, Mounika V., Surya "Lavanya", Shanu, Teena, Minakshi, Gudiya and Dhanya. Due to lack of qualifications, the Center voluntarily closed for staff training. As of 04.9.2024, most of these items had been obtained and classrooms were staffed with qualified providers.

This was fixed immediately. Plastic gloves and a bag were observed accessible to infants in an unsecured lower cabinet in the infant classroom. Gloves were observed accessible to infants on the infant changing table. Plastic gloves were accessible to children in the toddler room. Plastic gloves were accessible to the children in the DP classroom. The kitchen was unlocked and accessible to children. The kitchen contained a stove with electric burners. The burners were warm to the touch. This does not appear to be because the burners were on themselves but that the stove was on and warming to 400 degrees. A child could have opened the stove and burned themself. A taller preschool age child could have accessed the temperature control knobs for the burners. A secondary refrigerator/freezer was in the kitchen and accessible to children. The kitchen was locked by the provider within five minutes after a key was located.

Disinfectant spray was accessible to children at the diaper table in the toddler room. A gallon of bleach was accessible to children in the unlocked and unsupervised kitchen. It was locked prior to my departure. Disinfectants and cleaners were accessible to children in an unlocked and unsupervised janitor's closet. It was locked prior to my departure. The staff bathroom is not locked when not in use. It contains cleaners, disinfectants and plastic gloves. The fan or ventilation system does not appear to be functioning in the janitor's closet. The acting director indicated that the fan was not working in an adjacent classroom as well. As of 04.09.2024, a contractor to repair this has been contacted. Two storage rooms were unlocked and accessible to children. They did not appear clean and sanitary. Toilet plungers and toilet brushes were observed accessible to children in Developmental Preschool bathroom and toddler classroom bathrooms.

Door chimes or alarms were not present or functioning or present on most doors or on any gates. On 04.10.2024 door chimes were still not functioning on two doors. On 04.05.2024, this licensor observed preschool children and their providers for six minutes on the playground. During that time, two of the providers (Gudiya and Mounika R.)clustered together in one area in conversation with one another. Teena Joy remained on a tablet near the classroom doors, out of view of most of the children. For three full minutes, I observed all three of these persons with their back to a group of 10 children. During this time several of the children were using orange safety cones, at the pointy end, to dig holes in the ground, creating unsafe divets. One individual I believed to be a provider did appear to be circling the playground to provide active supervision. She appeared frustrated by the lack of active supervision being provided by the providers. She encouraged the children playing with the cones to find other fun and engaging activities. I later found out that this person is not a provider at all. She is an ABA therapist for one particular child. On 04.10.2024, this licensor observed several children playing unsupervised outside for four minutes.
