At the time of the onsite monitor inspection, there were plastic garbage bags in the top drawer of the toddler room changing station that were accessible to the tallest children present. At the time of the onsite monitor inspection, there was a fish tank wire that was plugged into a non-GFCI outlet, directly under the fish tank and accessible to the children present. (Green Room) At the time of the onsite monitor inspection, there was a refrigerator wire and pencil sharpener wire plugged into a TR/GFCI outlet directly over the children's handwashing sink in the Blue classroom, that was accessible to the children present.

At the time of the onsite monitor inspection, there was an emergency bag with a first aid kit in the reading loft of the blue classroom that was accessible to children via the installed wall ladder.

At the time of the onsite monitor inspection, the director, one program supervisor, and numerous staff present who did not have a current bloodborne pathogens training/review documented in MERIT or onsite as per labor and industry standard. At the time of the onsite monitor inspection, one Aide who was present and in ratio did not have a current first aid or CPR training in MERIT or onsite for review. At the time of the onsite monitor inspection, one staff member who was present and assiting kids with food did not have a current food workers card in MERIT or onsite for review. At the time of the onsite monitor inspection, 11 Aides had not completed childcare basics and had it documented in MERIT, most of which were past the 3-month window of being in the role.
