Licensor reviewed the child's records and verified the child had a documented allergy and Individual Care Plan on file. Interviews with staff determined the child's designated meal was not included on the food cart delivered by the kitchen staff. Staff reported that when the child's meal was not received, the classroom teacher located a container of yogurt in the classroom that was not labeled and assumed it belonged to the child. The teacher served the yogurt to the child without verifying its contents or contacting the kitchen regarding the missing snack. The yogurt contained dairy, a known allergen identified in the child's allergy documentation. Sufficient evidence was obtained to determine the staff failed to follow the child's allergy plan. Allegation determined to be Valid.
