at time of visit, there was insufficient ground cover under slides. Wood chip depth measured 3 inches and less on various playgrounds. Sufficient wood chips are on playgrounds; wood chips need to be spread out and maintained to ensure depth of at least 9 inches under and arounds slides and climbing structures.

Both staff persons report they were not standing near the climbing structure nor actively supervising the children when the child fell from the slide. Both staff persons were engaged in other activities, i.e. signing into the classroom and signing children out who had been picked up.
