“According to multiple staff interviews conducted onsite and a family interview, an infant was mistakenly given a bottle of infant formula by a staff member in the infant room without parental consent. At the time of the incident, the family had provided verbal authorization for the infant to receive breast milk only. The authorization was not documented in writing on the form used at that time. The staff member mistakenly provided infant formula to the child prior to feeding. The feeding was recorded on the feeding log. The formula offered was Kirkland Signature ProCare Infant Formula with Iron.”
