At the time of the onsite monitor visit, there was an a-frame climber and a jeep themed climb and slide toy on the preschool playground that had tires, balance beams, and a wooden arch bridge that were around the base of the climbers and did not have the 6' safe fall zone in each direction.

At the time of the onsite monitor visit, an outlet cover in the toddler room was broken and covered with tape, accessible to kids. Also, the sheet rock step under the toddler bathroom sink was broken/chipped, exposing the sheet rock. At the time of the onsite monitor visit, there was a laptop, glowing sphere, and speaker plugged into a power strip and long enough to reach the teacher handwashing sink (less than 2' away), that were accessible and in reach of the preschool children present.

At the time of the onsite monitor visit, the cabinet under the teacher handwashing sink contained Borax, a quart size container of vineger, a disenfecting spray bottle (with bleach water), all labled with "keep out of reach of children." The cabinet was unlocked and accessible to children present.

At the time of the onsite monitor visit, the preschool had multiple cords that could form a loop around a child's neck, accessible to children. Disco ball cord (4' long) under loft; air purifier cord (2'), extension cord around door (2.5' long). At the time of the onsite monitor visit, there were numerous plastic bags in the toddler bathroom that were accessible to children. At the time of the onsite monitor visit, the small group of cubbies by front right entrance gate was not secured to the wall an posed a falling/tipping hazard.

At the time of the onsite monitor visit, liquid soap and Ajax cleaner, and one cleaning spray bottle, were accessible to children in the bathroom cupboard. In the toddler classroom, there was a crate of 3-step cleaner bottles right next to a toddler handwashing sink, within reach of children.

At the time of the onsite monitor visit, the infant classroom had 7 children signed into the classroom (and were in classroom). The classroom is only licensed for 6 children. At the time of the onsite monitor visit, there were two children in the classroom that were over 12 months old. The classroom is licensed for children ages 3 months through 11 months.

At the time of the onsite monitor visit, three staff members who had been employed for more than three months (6 months to 2+ years) did not have a completed childcare basics training documented in MERIT.
