According to the interviews and video recording of the incident, Licensor is lead to believe that there was most likely than not intentional pinching of the child's cheek, there was also pulling of another child's arm that has occurred on the same video clip, and there was another time during the day when the same teacher pulled an arm of another child.

Licensor determined that the complaint is Valid. The provider enrolled a child with suspected fish allergy without completed ICP and the Lead and Assistant teachers did not prevent the child from ingesting suspected allergen.

The Storage room in school age classroom was unlocked. the room has vacuums, buckets and other janitorial equipment that needs to be inaccessible to children. 0260 (5) Toilet brushes were hanging on the wall with the brooms. The Director moved them into the janitorial closet.
