ALLEGATION(S): Two (2) Personal Rights allegations: 1. Facility staff yells at children. 2. Facility staff handles children in a rough manner. INVESTIGATION FINDINGS: On 10/17/2025 at 8:50am, Licensing Program Analyst (LPA) J.J.Z. and Licensing Program Manager (LPM) C.C. conducted an UNANNOUNCED COMPLAINT SITE INSPECTION. LPA met with facility representative (FR) Baishakhi “B.” H. LPA advised FR the nature of the inspection: a complaint regarding two (2) Children’s Personal Rights allegations were filed against the facility. Present during the inspection were seven (7) fingerprint-cleared staff and 41 children in care. Throughout the course of the investigation, LPA reviewed facility roster and personnel report. Interviews were conducted with Reporting Party (RP), teachers, children, and parents. LPA also made observations at the facility and inside the classrooms on multiple visits. Although the above allegatisons may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are unsubstantiated. Report was reviewed, exit interview was conducted, and Appeal Rights were provided to facility representative B.H. Notice of Site Visit was given and must remain posted for 30 days. ALLEGATION(S): Personal Rights - teacher yelled at student INVESTIGATION FINDINGS: On 08/23/2024, at 9:30am, Licensing Program Analysts (LPAs) C.U. & J.J.” Z. conducted an unannounced visit for the purpose of investigating a complaint for the above allegation of violation of personal rights. LPAs met with licensee/owner, C.B. Present during the time of today’s inspection was 38 children and 7 staff. This agency has investigated the complaint allegation that the a teacher yelled at a student. During the course of the investigation, LPA U. & LPA Z. conducted interviews with involved parties and potential witnesses. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted and report was reviewed with the licensee, C.B. Unsubstantiated Estimated Days of Completion:
