During the visit, the provider put her baby down to sleep in a bassinet that did not have a sheet. Instead, she had 2 blankets covering the mattress.

2(c): There was a string/shoelace hanging from a tree branch outside, accessible to children. 2(d): There was a garbage bag in a drawer under the nap mats accessible to children. There was also a garbage bag on the ground outside and the provider stated her children were playing with it. 3(a): There was a piece of metal that was not properly screwed into the exit gate, causing the screws to protrude and be accessible to children. There were multiple thorn bushes growing outside, that were accessible to children. The metal fence had pointy pieces of wire that were sticking out, accessible to children. 3(c): The bottom shelf of the diapering table was cracked and accessible to children. There was a door located downstairs that had a hole towards the bottome, accessible to children. There was a crack in a door upstairs accessible to children. 3(d): The fridge/freezer downstairs was accessible to children and did not have a lock/latch on it. There was an air vent upstairs by the sliding glass door that did not have a cover. 3(f): There is a tall shoe rack near the front door that was not very stable and was not secured to the wall. 3(g): The wire material on the chain link fence outside was very rusty. The handrail on the stairs had flaking paint. The walls downstairs had flaking paint. 4(f): There was a baby gate at the top of the stairs that was situated higher up from the stairs that created a tripping hazard and a gap larger than 3.5 inches.

The provider had several personal hygiene items sitting above the toilet in the upstairs bathroom. There was a bottle of hand sanitizer sitting next to the couch upstairs, accessible to children. 1(c): The provider had a bleach water in a spray bottle, but it was not labeled.
