Licensor observed that Assistant Teacher Christen S. did not complete the required 30-hour basic training within three months of her hire date, as mandated by licensing requirements. 0106 (3) (10) Licensor observed that Ms. Sabrina and Ms. Tia had expired Bloodborne Pathogens training certifications, indicating noncompliance with current health and safety training requirements. 0106 (11) Licensor observed that Assistant Teacher Christen’s First Aid and CPR certifications have been expired since May 2020. 0106 (12) Licensor observed that Ms. Sabrina and Ms. Tia’s Food Handler certifications expired on August 15, 2024 Ms. Christen since 09/03/2017. 0106 (8) Licensor observed that Ms. Sabrina, Ms. Tia, and Ms. Christen had Safe Sleep training certifications that expired as of April 27, 2024.

Licensor observed that two large wooden panels had become detached from the frame structure and were found lying on the ground. As a result, the back alley area was left exposed and not properly enclosed, failing to deter unauthorized individuals from entering the premises. 0145 (11) Licensor observed personal home furnishings stacked and stored in the outdoor play space, along with wood debris leaning against the wooden fence. These items were accessible to children during outdoor play. 0145 (6) Licensor observed that the entrance gate door was missing, creating a significant safety and security concern. This condition allows children to exit the premises unsupervised and permits unauthorized individuals to gain access to the property.

During the observation, Licensor Harris observed four wooden planks at the fence near the shed that were pushed back and out of line of the fence. The gaps was bigger than 3 1/2 inches. (8)

During the inspection, Licensor Harris observed playground equipment of five slides. At this time, Licensor Harris observed four sildes that are not within a 6 feet falling zone range. Licensor measured two slides (taller slides) with a fall zone of 56 inches from the slide to the wooden post and 58 inches from the ladder to the wooden post. There were two additional slides that are shorter. The falling zone from the ladder to the wooden post is 60 inches for both slides. The end of the slides are placed in front of each other with a falling zone of around 30 inches. (1) During the inspection, Licensor Harris observed pea gravel under playground equipment (5 slides) in the backyard area that does not have a required depth that means CPSC standards. Licensor measured pea gravel at 1 inch in depth. (1)(b)(i)
