A toddler slide was placed over carpet inside the child care.

The water in the bathroom was tested to 131* degrees. The stairs leading to the outside deck did not have a baby gate placed at the bottom. Some window blinds were observed to have cords that were capable of forming loops. The sliding glass door that leads to the back yard did not have stickers or artwork at children's level. Two outlets in the "indoor gym" did not have tamper resistant covers on them.
