Licensee Valentina Tyukalo and Lead Teacher Yulia Bondarchuk have not completed BBP training since 2021. At the time of this licensor's unannounced monitoring inspection on 09.30.2024, the licensee did not have current Safe Sleep training. She obtained one later that day. Yulia B. does not have current Safe Sleep training. She has taken it since 9.30.2024. Neither the licensee nor Yulia B. had a current food worker card on 9.30.2024. The licenses has completed food handler's since 9.30 but Yulia B. has not.

The provider has a dome climber. The climber is not placed six feet from a wooden barrier that encircles the play area. The playground wood chips are not nine inches deep six feet surrounding the climber. As of 10.7.2024 it had been moved and additional wood chips had been put in place.

The provider made a fire inspection request this Summer from her local fire authority. They declined to come but she did not complete the WSP form herself.

The provider does not have a system in which she solicits for and updates children's information annually. Therefore, children who have been enrolled for more than one year do not have current immunization records and they are likely outdated.

On 9.30, the diaper creams and ointments stored in the bathroom are not labeled with children's first and last name and were accessible to children in the bathroom drawers. They are not accompanied by medication authorization form. As of 10.7 they had been made inaccessible.

Upon my arrival on 09.30.2024, I observed 12 children in care ranging in age from two to five years of age. The children were napping. The provider was working by themsleves as her assistant was "on a break" off the premises. She immediately texted the assistant who arrived at the child care in ten minutes time.

The Disaster/Emergency policies/plans are not reviewed at program orientation, annually with all early learning program staff with documented signatures, and when the plan is updated. The policies/plan does not include: How the early learning provider will evacuate children, especially those who cannot walk independently. This may include infant evacuation cribs (for center early learning programs), children with disabilities, functional needs requirements, or other special needs. How the provider will maintain the required staff-to-child ratio and account for all children How a provider will account for all children following an earthquake. How a provider will coordinate with local or state officials to determine if the licensed space is safe for children after an earthquake. How parents or guardians will be contacted after the lock down emergency ends.

Dome climber does not have playground chips underneath that are nine inches deep.

Plastic gloves were observed to be accessible to children in an unlatched/locked drawer in bathroom vanity. The stairs to the unlicensed portion of the home have a gate but the gate was not in place when this licensor began the inspection. Electrical cord for keyboard was observed plugged into outlet cover that turns a two outlet into a four outlet one.

Cleaners were accessible to children in the vanity in the bathroom and in the garage which was unlocked/unlatched.

The front door and garage door used to be alarmed but the alarms were not functioning today. The sliding glass door to the backyard was not alarmed and does not make a noise when opened. This licensor observed the provider's dog "kissing" a toddler age child by licking his mouth and neither the provider or her assistant were in the room. I called the animal away from the child.

Nataliia Shyshak has not completed Child Care Basics and was working alone on the premises with 11 children when I arrived today. Although Tyukalo reports that Shyshak has completed Safe Sleep training she was unable to produce verification of that today and it is not showing in MERIT. Although Tyukalo reports that Shyshak has a current food handler's permit, she was unable to produce verification of that today and it is not showing in MERIT. Shyshak assisted Tyukalo with serving the children lunch today. Neither Nathasha Rosnoslik nor provider Valentina Kyukalo have completed BBP training on an annual basis. Valentina Kyukalo's Safe Sleep training expired in 2022. She has not served an infant since that time so did not think she needed to renew her training.

When this licensor went outiside to retrieve something from my car, I observed that the deadbolt to the main front door was engaged and it did not automatically unlock when the doorlatch was engaged.
