Licensor observed plastic bags accessible to children in the bathroom used by the Preschool classrooms. Provider removed the plastic bags during licensors visit. Licensor observed the slides on the climber located outside the Sunshiners classroom have bolts sticking out on both sides of the slides. The caps covering the bolts were missing. Licensor observed several trucks and toys in the outdoor licensed space that were cracked and broken. Licensor observed a cracked and broken plastic storage bin in the outdoor licensed space off of Rainbow. Licensor observed the cubby just inside the Dewdrops classroom was not anchored to the wall. Licensor observed metal trucks in the outdoor licensed space that were rusty and had flaking paint. Licensor observed the garbage can in the Stargazers bathroom was rusted. Licensor observed the electrical outlet next to the sink in the bathroom was not an tamper resistant GFCI type. This was observed in the bathroom of the Stargazers classroom.
