There was only one disaster drill documented on the drill log form and it had a date of /16/24. It was unclear what month the drill occurred and when provider, A.G. was asked by licensor, Faiza Ibrahim provider, A.G. could not recall the date she conducted that drill. Licensor informed her of the disaster drill quarterly completion requirement.

4 children enrolled. 4 files reviewed: 1 child file was missing the 2nd page of the permissions form, 2 child files were missing the medical consent form/2nd page of enrollment registration form, 1 child file did not have immunizations and another one had an incomplete registration form. During the 8/2/2023 a complete regisration form missing medical consent was cited at this visit.

Interpretering was done by licensor, Faiza Ibrahim during the visit. During the visit the licensor asked to see files and this licensor asked if her son still lived in the home. Asha responded which one and the licensor responded the one in MERIT. Asha stated that she was the only person that lived in the home but that she put the son down because he comes by sometimes during child care hours. It was unclear if more than 1 son lives in the home or had lived in the home by Asha's response. When discussing the assistant's file and what missing the licensor stated that her assistant was missing TB test results. Asha yelled she sent them and then yelled that she was not her assistant she was a volunteer. The licensor told her to update MERIT then because she had put her down as an assistant in MERIT and that what the licensor was requiring is also required for volunteers.

The assistant, UA did not have documentation of TB test results in her file. This was cited during the 8/2/2023 inspection. On 8/15/2023 via email a TB test appoinment sheet was sent to the licensor but not the results.

In the corner of the main space where the high chair is located a cell phone and IPAD were plugged up. The cording for the equipment posed hazards. The IPad was on plugged in and sitting on the seat part of the high chair and the phone was on a nearby table. We arrived to the home at 10:53a.m. and Asha started cleaning up and placing items on the stairs behind the gate. At 10:55a.m. Asha removed the items but the child according to Asha had arrived at the child care at 9a.m. The mutiple rugs stacked on top of each other forms a tripping hazard. In there is visible chipping paint on the wall. It appears the paper towel holder may have been placed in that space and then moved over. Several times during the visit Asha's voice was escalated when communicating with licensors.

When checking the refrigerator and freezer of the space provider, Asha told licensor, Faiza to inform this licensor that the families bring their own food. The licensor then inquired on where the child's food was after looking around in the space. According to Asha the parent is going to drop it off. The child had arrived at 9 and we left the home a little after 12:00p.m. no food was observed. What was observed when we arrived was a bottle sitting on a shelf. The child in care is 2 years old.

Infant and toddler sleep, rest, and equipment · WAC 110-300-0290(3)(c) · Short Term
The provider, Asha has added a thicker mattress to the Pack N Plays. One is in the bedroom and one in the main space. The added mattress is not snug fitting cited (8/2). The mattresses are also not moisture resistant and the one in the main room was observed with stains. Licensor inquired to the provider,Asha via licensor, Faiza on where the sheet and moisture resistant barrier was for the foam feeling mattress she replied they were in the wash. The licensor checked the other Pack N Play which was in the Bedroom and it did not have a moisture resistant covering on it only a sheet. The mattresses that the provider also wants to propose to use for Overnight Care did not have a moisture resistant covering on them.

Cleaning schedules · WAC 110-300-0241(1)(a) · Short Term
The program has a cleaning schedule but does not appear to be following it. The kitchen had an unpleasant smell. Later during the visit the garbage that was taken about 20-30 minutes after we arrived. The garbage was full located under the kitchen sink in the cabinet was full. There were pots and items on the stove and by the stove including packaging close to the stove and a little piece of paper on the stove eye which the licensor removed. The pots had substance/food items inside of them but it did not appear to be the food that was to served today for the children's lunch. The table used for eating was observed to have a plate of food on it that was not being eaten. It was removed and the table was never 3 stepped washed rinsed and sanitized afterwards despite there was visible debris on the table. Later the assistant, Umulkheyr announced it was snack time and the table had debris which was observed and noted. The high chair nor the table was observed to be washed rinsed and sanitized prior to eating. Possible smell may have been from the garbage can but not sure. The garbage can was really full and did not appear to have been emptied from day prior.

The assistant stated she had prior work history when asked but when reviewing MERIT there is no work history listed. The TB/X-Ray provided was from 5/21/2018. According to the provider and the assistant she recently started working at this location and has not been working for a week.
