During the inspection, this licensor reviewed five child files. The file for S.M. contained a current immunization record labeled "Not Complete". A signurature from the child's guardian stating they would update the file within thirty days was present, however, more than thirty days had passed since the form was signed by the guardian. Basketball hoop has nothing in the base of it to secure it. There is a little teeter-totter in the yard that is on grass.

Secure first aid kits and other heavy items high up on shelves. Back porch was observed to have chipping paint on the handrails around the porch.

Fire Marshal had not been started/completed thus far for the year. The licensor provided a copy to the provider for completion. Clean vent in the restroom.

Front entrance door and sliding door were both locked during the visit. The front door has a deadbolt that was locked and had to be unlocked in order for the licensor to exit the space.

The broken gate outside has pieces that are pointing back into the play area and are sharp at the end. The water tables are right next to this broken gate. The basketball hoop on the covered porch is a tipping hazard. Outlet in the restroom was observed to be GFCI but not tamper resistant.
