The provider's teenage son assisted the provider by supervising the children for approximately 20 minutes while she warmed lunch. He could be heard audibly ssshing children who were speaking in slightly louder voice and telling them "no" while they got up from chairs while waiting for lunch. He does not meet any of the qualifications of an aide, volunteer or assistant. He has only a t.b. test and portable background clearance. The provider has not completed BBP training on an annual basis as required by Washington State Labor & Industries. The provider's food worker card expired on 11.09.2024.

A stick was in place blocking exit from the sliding glass door fire exit at the rear of the home. This was removed prior to my departure.

The provider has not requested an annual fire inspection or conducted her own inspection using the state fire marshal form. A propoane tank in an unlocked shed in the outdoor play space was observed accessible to children.

The provider does not have an Individual Care Plan for a child who is severly allregic to tree nuts. The provider reports the child has an epi pen at home but not on site.

Two of six child files reviewed do not contain immunization records at all. Four files contained out of date immunization records as the provider has no process whereby she updates this information annually and the children's original records are more than one year old.

The provider stated that children had played in the licensed outdoor play space today. This licensor observed an electric saw immediately inside the door in an unlocked shed. She did not realize it was locked. The shed was locked prior to my departure.

Clorox sanitizing wipes are accessible to children in an unlatched cabinet under the bathroom sink. These were moved while I was present.

The provider's mother came to visit beginning in December 2022 and a portable background clearance was not completed. One was completed during my visit. She also did not complete t.b. testing or screening. The provider created an individual profile for her mother today in MERIT but was not able to complete the portable background check application. MERIT support was e-mailed regarding this difficulty.
