During onsite visit and recheck, provider stated her landlord and his wife live upstairs. They do not have a PBC or negative TB test on file.

A TV cord plugged into the wall, was observed accessible to children posing a risk of forming a loop around a child's neck. Children's cubbies noted loose with a screw coming out of the wall and observed no longer securely anchored to the wall. Unsecured cubbies pose a risk of tipping over onto children playing or sleeping below. Fencing in areas had worn wood slats observed with splintered edges, all accessible to children posing an abrasion hazard. Water temperature in the bathroom tempted out at 125 degrees posing a health risk to children.

Little Tykes single use trampoline noted in the yard accessible to children observed as bouncing equipment posing a safety risk to children.

Storage room observed with black and white spots along with peeling paint to the ceiling and black spots to the corner wall. A strong smell also noted by licensors that could be smelled in the licensed areas as well as the storage room. The Lead Teacher reported that there was a past water leak that had been repaired. The owner of the home arrived onsite and reported that any leak would be fixed and the damaged drywall replaced.

The floor plan posted, does not clearly show where the fire exits or rooms are. Only one exit door out of 2, has arrows pointed to identify it as an exit. The floor plan viewed posted on the wall, does not represent the actual layout of the downstairs area.. The provider has a disaster plan developed, but is not following the attendance policy for face and name check. The attendance was not current during the visit with only 2 children out of 5 accounted for. The electronic attendance was incomplete and no paper attendance was taken as reported by the Lead Teacher left in charge.

A car with 2 Daycare children and an adult was observed with the adult backing out of the driveway. 2 licensors requested the person to stop and identify who she was. The Family Home Aide Identified herself and said she was taking the children home. 2 children observed being transported in the car with the Family Home Aide with only one in a carseat. A 4 year old girl, was observed secured fully buckled in a seat belt and not a carseat or booster seat. 2 children in the car observed being transported, did not have any enrollment paperwork that would include emergency information and emergency medical authorization forms accessible for each child being transported. The 2 children in the car, had no enrollment paperwork onsite as it was reported that the mother had the paperwork with her.

2 emergency exit doors were observed leading to the same outside play area and pathway as the fence has only one gate to exit the yard.

Family Home Lead Teacher who was in charge, reported that she does not know where the staff files are located. Licensors were unable to view required documents for current annual BBP training and for CPR/First Aid training. In Merit, not all staff were verified complete for the trainings. Family Home Lead Teacher in charge during the owners absence, does not know where staff files are at to confirm that Food Handlers training has been met for staff. 2 out of 3 teachers were confirmed verified in Merit to have had the training. The Lead Teacher who was observed making lunch was observed serving food to children, could not be verified in Merit complete. No paper documentation onsite to verify that the Lead Teacher had taken the training. Lead Teacher has not taken Child Abuse and Neglect training as reported in Merit. No records were onsite to verify that the training was completed. Lead Teacher and Family Home Aide have no record in Merit that Medication Management Training was taken or records onsite that verify it was taken as approved by the department.

Storage area (room) observed not clean and sanitary. Room where cots, crib, and various daycare items viewed stored, white and black spots were observed on the ceiling and as well as black spots observed on the corner wall. Caster oil hair product, skin type lotion, and tubes of toothpaste, all viewed accessible to children in the bathroom.
