Assistant A.G. has expired Safe Sleep Training and provider, R.B's Safe Sleep is self-entered and not verified. Provider will need to follow up to find out why class is not correctly noted in the system. Certificate was observed in the file.

Bottle did not have the child's first and last name. The kitchen area has shelf paper/contact paper on the counters and in some areas it is torn. The shelf paper/contact paper needs to be re-applied to the entire counter or removed and will need to ensure that the surface/wood/laminate under the contact paper is sealed and cleanable to meet the WAC of moisture resistant and sealed surface.

Need to ensure that slide is spaced a least 6 feet from other equipment. The slide was observed too close to other equipment. Need to add more woodchips under the slide structure. Ensure that debris, litter plastic wrapper from water (picked up when we were in the yard) and other items are not present in the yard. Remove weeds from yard. Make sure that hose that was in the yard then put outside the yard is rolled up on a holder or put away to ensure that it is inaccessible. Those poses a tripping and strangulation hazard.

Need to ensure that the printer and computer equipment on kitchen counter is secure, secure shelving unit (white) with dress up clothes is secure it currently flimsy (moveable). Book shelf that is located in front of the fire place door as a barrier is moveable. In the restroom there is chipping paint chunks of wall gone in some area. On shelving units there is contact paper that is peeled off and peeling. In the restroom the outlet has to be GFCI and Tamper Resistant.

Nap Room where cribs are located does not have two exits. The exit to the front door is blocked by a couch. The window in the room is too high to be counted as a potential exit.

Fire Marshal Form was not observed and will be sent to the provider to use. Fire place which is blocked by a bookshelf was warm/hot to touch. The provider thought the fire place was turned off completely but when the licensor touched the back of the bookshelf and the glass door of the fire place it was hot to touch. It appears the pilot light may still be on which is allowing the head radiation. The fan in the restroom needs cleaning. Smoke detector is needed in main childcare space. This is another sleeping area for children.
