There is an apple tree in the next door yard and the several apples have dropped into the yard several look rotten/bruised and there was a bitten off apple in the yard and thrown in the yard. The apples need to be cleaned. A broken hula hoop was in the yard. The wooden gate nailed in wooden panels are coming away from the wooden gate/frame which was enabling the gate to self-close. The hardware may also need some adjusting/tightening to ensure closure.

The climbing structure which was cited and discussed during previous visits is now on woodchips but the woodchips are not 9 inches deep. The woodchips are less than 1 inch deep. The woodchips have been poured on the grass only in the slide slope area in front and on sides. There was not a 6 feet fall zone all the way around the equipment with adequate ground covering of 9 iinches deep. Basketball hoop forms a tipping hazard because there is nothing in the base of the hoop.

Back door that leads to play area and is a room where the children play was locked via dead bolt. Licensor unlocked the door on way to check the play area and informed assistant, HH upon return to leave unlocked when children present. HH relocked the door shortly after the licensor walked into the other room (could be heard being locked).

The grass that was in the yard requires cutting. The play materials require deep cleaning. There was two stand alone slides and a slide that was a play structure and a teeter totter none of the equipment was on required ground covering it was all on grass. The items were close together ranging from approximately 1 foot to 4 feet away.

Black crib in room was placed by a window. Hardware for cording for curtains was observed to be sticking out of the wall. The curtain hardware to open and close the curtains has been removed from the hardware on the on the wall and placed at the top of curtains. But the hardware is still there. Bookshelf and kitchenette needs securing the items were on carpet in the main area which made them unstable. Chipping paint on cribs. Television cording is loose from the wall. Air Condition cording in the napping room was loose/not plugged into outlet. Bathroom lighting did not appear to be shatterproof. There was no packaging present for the licensor to review to confirm if it was shatterproof. Provider and sister were unable if it was shatterproof. Outlet in bathroom by the changing table was not GFCI but was Tamper Resistant. In the kitchen there were outlets that were tamper resistant but did not appear to be GFCI

Front door and back door which are both exits were locked while licensor was there. The door has a deadbolt. Door is required to be unlocked during the hours of childcare an/or door can be changed. In the napping room there is a window that currently has a air condition securely in the window. There are screws present that is securing the air condition in the window. In order for the room to be licensed the air condition needs to be removed to allow for an exit. Once the air condition is removed from the window the window must remain unlocked. If the air condition is not removed from the window the space/room cannot be licensed.

During the visit and arrival to the home the licensor observed a car parked up the driveway area by a detached garage/dwelling space. When heading to the yard to inspect out the back door it was confirmed that the dwelling is occupied by another tenant. The space is on the property/premises and there is no separation. Once you walk out the back door and get down the stairs you are at the door of the other dwelling/rented space. The tenant whom is on the premises/property does not have a background check. Tenant(s) are required to complete a DCYF background check and submit documentation of a TB test.

Provider's file was reviewed and she did not have copy of CPR/First Aid, Food Handlers or BBP for review. Provider talked about it being in MERIT and the licensor informed her that she needed to see the documentation in order to verify. The licensor also explained that First Aid had been input with a date of 11/29/23 -11/29/25 but that it was not verified so the licensor needed to see it. The Food Handlers Permit in MERIT expired 1/2/2024 but was verified. But it is expired. CPR expired on 9/5/2023 in MERIT and BBP expired 9/5/2022.
