Room 119. the smartboard had loose cords. Room 113, had a charger plugged in and the cord was hanging and accessible to the children. Room 114, had a charger cord near the back window that is accessible to the children. PLAYGROUND: The play structure with the blue slide closest to the building has orange chipping paint. The 2 seated structure with steering wheels has rusted screws and chipping paint. The "store" sign near the back fence has green chipping paint. The posts in the back fence have green chipping paint. The gate along the back fence has nails sticking out.

In room 119, there was a epi pen box that did not have the authorization form. The box for the epi pens had an expiration date of 9.2024 and the acutal epi pens had an expiration date of 3.2025.

Rooms 107 and 114 had cables cords hanging from the screen and they are capable of forming loops. There were plasic bags accessible to the children in rooms 107, 114 and 119. The yellow handles on the wooden play structure are rusty.
