Two sets of gardening hose in the outdoor area were observed to be accessible to children and were capable of forming a loop around a child's neck at the time of inspection. A piece of equipment with strings attached to it was observed in the infant nap room that was accessible to children and the strings were capable of forming a loop around a child's neck at the time of inspection. Holes on the wall were observed in the bathroom area that were accessible to children at the time of inspection.

The annual fire safety inspection report was not observed on site at the time of inspection. This licensor was unable to determine the working condition of the fire extinguisher because no inspection tag was attached to it at the time of inspection.

At the time of the onsite monitor visit, this licensor observed medications (pinworm treatment, antibacterial spray, cough/sore throat syrup, ear ache medication, and pink pismuth) stored in a plastic first aid box on a bottom shelf in the living room that was accessible to children in care.

At the time of the onsite monitor visit, this licensor observed a cord for an air purifier in the living room that loose and able to form a loop around a child's neck. At the time of the onsite monitor visit, there were multiple plastic bags present on bottom shelves in the kitchen that were accessible to children. At the time of the onsite monitor visit, this licensor observed a fishing knife in an open face kitchen cabinet, approximately 5' high, which is an area accessible to the older children prensent. At the time of the onsite monitor visit, this licensor observed a sound machine that was on a window sill and had the cord hanging down within reach of an infant, posing a falling hazard. At the time of the onsite monitor visit, the kitchen ceiling was flaking and cracked. The kitchen is accessible to children in care. At the time of the onsite monitor visit, there was a power strip accessible to children in care in the living room.

At the time of the onsite monitor visit, the cabinet under the kitchen sink was unlocked and contained dish washer soap. There was also a basket in the hallway next to the kitchen that had a bottle of swiffer cleaner. (Both marked keep out of reach of children and both accessible to kids.) At the time of the onsite monitor visit, there was an electric weed wacker present in the backyard spacee accessible to children.
