At the time of this inspection licensor observed the attendance record was not available onsite for department review. Lead teacher U.A. stated he was not able to find the tablet used to record electronic attendance. TA provided to use a paper record as a back up method to ensure enrolled children are signed in and out of care per WAC requirements listed in 0455(2).

At the time of this inspection, licensor tested the emergency exit gate from the backyard leading to the front of the home and observed it was not self-latching. Licensor observed the latch hardware was not installed onto this gate and it was not able to fully close.

At the time of this inspection, licensor audibly heard a deadbolt on the front emergency exit door being unlocked upon arrival and observed the emergency exit sliding glass door leading to the backyard was locked from the inside. TA provided and violation was corrected onsite.

At the time of the onsite monitor visit inspection, there was a plastic, spring, bouncy horse in the yard that did not have CPSC safe fall zone material underneath. (placed on grass) At the time of the onsite monitor visit inspection, the plastic, spring, bouncy horse did not have a 6' fall zone around it; other toys were all around it.

At the time of the onsite monitor visit inspection, there was a jar of vasoline on the chaging table that was accessible to children and labled with "keep out of reach of children."

At the time of the onsite monitor visit inspection, there were numerous broken pieces of plastic from old toys throughout the yard, that were smaller than 1.75" x 1.75" x 1.75" and posed a choking hazard to infants and toddlers who use the yard. At the time of the onsite monitor visit inspection, there was a plastic bag in the cabinet under the sink in the bathroom that was accessible to children and posed a suffocation hazard. At the time of the onsite monitor visit inspection, the water temperature was measured to be 125.7 degrees Fahrenheit.
