Mats used for overnight care was observed to not be moisture resistant/water resistant. The zipped covering for the mat is made of cloth and the mat is cloth. There is not a barrier on the mat or over the mat that is moisture resistant/water resistant. The mats were stored in a closet area and were touching. There were other overnight mats that were still enclosed in plastic packing but they were stored on top of the mats that are currently without a barrier between and being used for the 3 overnight care children.

Little play children kitchennette is moveable/tipping hazard. Restroom water temperature is at 138.4. Outlet in restroom by the is tamper resistant but not GFCI. Children were in unlicensed space and the outlet by the window and couch in the space is not tamper resistant. Children were oberved in the kitchen and the outlets in this space is also not tamper resistant.

Licensor did not have access to F.M's file during the visit to view and verify documenation. Provider, FA had an old Safe Sleep certificate in her file that read 6/24/2023. The Safe Sleep in MERIT is Self-Entered and not verified with a date of 6/17/2024-6/17/2025. Provider will need to follow up with MERIT regarding the unverified training.
