Provider does not have attendance present for children to be reviewed. The tablet is reported to be working glitchy needing to be plugged in and no paper attendance has been taken per provider. Even with the tablet plugged in, the tablet was not working to view the attendance. No records of parents signing in or signing out their children either electronically or on paper. No record of who is picking up or dropping off children. No records of staff attendance observed.

Back exit door was not able to open more than about 10 inches as carpeting that had been added, was blocking the door from opening wide enough to allow someone to walk through. The door needed a good shove to unstick it and to close the door. When the provider corrected the noncompliance to remove the carpet, 2 throw type carpet were observed stacked on top of each other.

Provider does not have a smoke detector/carbon monoxide detector in the sleeping area, only one is noted in the hallway.
