Status says in progress for some teachers who have been employed for years. 10 hour annual training was not completed in 2023, however provider needs to ensure that 10 hour annual training for 24 is completed by the end of the year. Provider needs to update Merit when trainings are completed.

On 5/14/24 this licensor was notified regarding a concern with a staff member. E.B's interactions with children that was observed via classroom video footage. The licensor received the video footage via email, reviewed the classroom video footage and informed the director, K.S to call incident in. In the video it was observed that staff member E.B had inappropriate discipline/behavior management interactions with toddlers in the room. E.B. was observed firmly grabbing children off the table and placing the children firmly and forcefully on the floor. Staff member, E.B was terminated from employment on 5/15/2024. Licensors also conducted an onsite visit on 5/30/2024 to follow up with staff that observed the incident via the classroom video footage. Program Supervisor, O.W was in the office along with the other teacher from classroom, M.B (whom was on break) and observed the interactions with the staff and children via the live video footage screens in the office. K.S was notified and then followed up with the licensor for next steps.

During onsite inspection it was concluded that video footage was observed by leadership staff which confirmed inappropriate interactions between staff and children. The first incident in January the staff was observed picking a child up by her hand/arm which resulted in a re-occurring nursemaid elbow injury. The second incident in February the staff was observed pushing a child while on a cot which resulted in the child hitting their head on the floor or the side of the cot.

Licensor followed up with referents and conducted an onsite inspection at the site. In January a staff member picked up a child inappropriately and the child sustained nursemaid elbow which was observed on video by the leadership staff. In another incident in February the same staff member pushed a child causing the child to roll/fall off the cot and hit their head on the floor or the side of cot which was also observed on video by leadership staff. There were no reports of the incidents made to the licensor and/or the CPS intake line until later February.

An onsite interview was conducted with Program Supervisor, M.P.; Owner, K.L; and Director, K.O. regarding inappropriate incidents observed in the classroom by the Teacher, A.F. A phone interview was conducted with teacher, F.P. The licensor was also able to observe 4 classroom videos that displayed the interactions between staff member, A.F. and children. Information gathered and observed confirmed inappropriate interactions and behavior management between staff and the children.

Prior to going to the site for a monitor visit the licensor checked MERIT on 10/24/2023. Staff were noted in the MERIT system to have withdrawn background checks. On 11/30/2023 this licensor reviewed the sytem and staff still did not have a clearance. On 12/4/2023 the licensor reviewed the MERIT system and staff member: J.B (D.O.H.-11/9/2023); C.M (D.O.H.9/26/23) still did not have a background check clearance. The status read withdrawn. There was a new hire K.C. that also had an withdrawn PBC. The licensor also followed up with the owner, Kyla regarding her background check which was expired and she was not attached to the site MERIT page. On 12/7/2023 this licensor followed up with owner, Kyla regarding background checks. Some staff that were discussed earlier in conversations have now been cleared. However, the following are still not in complaince: J.B's Background Check is withdrawn, C.M's Background Check is withdrawn and owner, Kyla's reads expired but was resubmitted on 12/5/2023. Staff are required to follow up and follow through with all requirments from the Background Check Unit to obtain a clearance by December 22, 2023.

This licensor followed up with the parent and the program director and confirmed that the child does not know how to roll over. The staff in the room at the time could not remember how they laid the child down. The parent arrived for pick up shortly after the child had been laid down. It appears since the child cannot roll over that the child was laid down on their stomach and not on their back as required by WAC.

All rooms were observed to have chipping paint, peeled paint, and peeling paint on walls and/or in window sills. Rooms throughout the entire site have unsecured home center/kitchenette equipment that is not secured. There is also shelving units that have wheels on them in the rooms throughout the site and they are not sturdy/secure. Room 201 had a cracked/broken on the side kitchenette sink. Teacher work stations (shelving units) throughout all rooms observed to not be secured. Room 303 had plastic bags in the cabinet which was accessible to the children. The restroom also had plastic bags which were accessible to children. Room 304 had a floor lamp that was not secure. Several rooms with blind cording hardware that is no longer attached to the wall to enable loop access is no longer affixed to the wall and forms a hazard for children. Room 301 had glass items and little speakers stored on a wall on shelves by the restroom that was not secured. 301 also had Glade Spray in the unlocked teacher small cabinet/work station. Several Room observed with cording from equipment/chargers/speakers that were observed hanging and dangling in rooms and accessible to children. Mop/Floor Steamers are observed stored in diaper areas that are currently not being used for children and the area is freely accessible to children. Cording for the mops can be accessed by the children. In room 202 the water is slow flow so an adequate read was not able to be obtained for the hot water. The cold water had a steady stream but the hot did not.

Staff were observed via MERIT missing Medication Training. Medications were observed to not be labeled with chidren's first and last names. Need to also ensure that parent/guardian fills out forms in accordance to WAC and Health Care Policy. Medications was observed in room 202 for a child to have Motrin but the forms for the medication could not be found. Staff stated the child had not been given the meds in months. Informed to return to the parent. In Room 202 Aveeno lotion was observed in a cabinet accessible to children.
