Entrance and exit door for the family home was locked via the deadbolt during the visit.

Fire Safety Inspection Form was not completed and was provided during the visit. Restroom fan needs cleaning. Smoke/Carborn detector was not observed in the main space where children sleep and a school-age child was observed sleeping. The smoke/carbon detector was observed to be in the hallway by the kitchen.

Spray bottles for the restroom are accessible to children. The latch for the cabinet needs to be repaired in the restroom and spray bottles can be placed in this location to ensure that they are inaccessible. Broom was on the back porch that leads to the outside area and accessible to children.

There was no documenation that the handbooks had been signed by parent/guardian. There was no documentation that the staff reviewed the disaster plan handbook. Drill log observed was completely blank did not have the year for the form either. There were no monthly inspections, monthly checks, monthly fire and evacuation drills nor quarterly drills documented on the sheet. According to enrollment form for children some started in January of this year.

Cording from television and gaming system was loose and could form a loop. Metal pieces (rods) for the chainlink fence was in the yard leaning against the chain link gate accessible to children. The restroom water was 120.6 degrees. The outlet in the restroom was GFCI but not tamper resistant.
