Child's enrollment paperwork reads that the child has asthma. But there is no asthma plan nor paperwork in the child's file. According to staff the child's parent notifies them each day if the child is having some problems with their asthma. But there is no medication or paperwork on site for the child. If medication is not needed at the program then a letter needs to be obtained from the child's doctor.

Cording from equipment in the napping/sleeping room needs to be secure to the wall. Secure homecenter and any furniture that is not stable and secure on the floor. Secure television.The outlet in the restroom has GFCI on it but does not appear to also be tamper resistant visual shutters were not observed nor does the outlet have any verbiage that indicates it is TR. Outside there is a nail protruding from the wooden fence by the exit. (shown to H.O.)
