Provider reported the 3 yr old child arrived at her daycare with a black eye and when provider asked the child what happened said the mother hit her. Provider reported the injury on 2/20/25 – see Intake 53632190. Provider acknowledged seeing the blisters on the infant's fingers on his left hand, to the CPS investigator, the Kent police officer and the infant's parent but did not report the injurys to CPS or DCYF Licensing

Blankets were observed to be used in a pack' n play and on a swing when they were occupied by sleeping infants at the time of inspection (The violation was corrected on site).

A see-saw was observed to be placed on grass surfacing at the time of inspection (The violation was corrected on site).

A tight-fitted sheet was not observed for the mattress being used by an infant at the time of inspection. Mulitple pack n' plays were observed to be arranged side by side that were not spaced at least thirty inches apart at the time of inspection. An infant was not immediately removed from the swing and was observed to be sleeping on the swing for an extended period of time at the time of inspection (The violation was corrected on site).
