The individual care plans observed did not meet the following requirements: (a) Some plans did not identify foods that cannot be consumed. (b) Some plans did not specify foods that can be substituted. The forms were not signed by a healthcare provider. Some forms lacked complete information regarding reactions, treatments, and end dates. Not all medications provided for children with allergies were accompanied by the necessary forms. 110-300-0186(2) - A child, in #102 classroom, that have an allergy did not have medication provided by their parents.
