Provider was not present during the visit but provided guidance to staff and household member regarding location of file documentation. Licensor was not able to find documentation and after reviewing file a few times the binder was given to Q.M, H.A and A.H to review the binder to locate missing documentation. MERIT reviewed and files for staff. H.A. did not ahve docmentation reflected in MERIT nor in the binder of Food Worker card, Safe Sleep. Basic STARS. H.A. did not complete Basic STARS nor the required health and safety trainings via the portal. Food Handlers for provider was not observed in the file for verification. In MERIT there is dates for a food worker card but a card was not observed in the file for review. Assistant M.A had expired BBP and did not have documenation of Food Handlers in their file. Safe Sleep is also expired and expired in August. A.M did not have documenation of Food Worker card on file nor Safe Sleep Training. Assistant A.H. did not have documenation of Food Worker card.

Assistants that do not have basic STARS trainings did not have documentation of this training. Children wipes were on the changing table lower shelf and accessible to children. Assistant, A.H was asked to move them by the licensor during the visit.

During the visit the the provider was not present until the licensor was almost done with the inspection. The program did not have a complete first aid kit. The licensor did not observe the emergency car kit nor a first aid kit for the file. Children files were incomplete with medical consents so if the vehicle has copies of the files they are incomplete because the ones on the premises were.
