Children were seen to get up and down from the table constantly throughout snack, playing with items, touching many surfaces. They did not wash their hands before returning to the table or when leaving the table.

2a) Magnatiles are seen stored in buckets on bottom shelves, accessible to all children. The magnets pose a risk. 2d) plastic bags are seen under the bathroom sink, accessible to children. 3c, 3g) a plastic tractor outside was seen to have broken pieces to include a steering wheel. A toy purse was seen to have ripped plastic hanging from it. A pile of stuff was outside against the house on the east side, accessible to the children. folding chairs outside were seen to have ripped seats. 3f) many items are seen stacked up on shelves causing them to lean. The items pose a risk of falling. The cubbies are not secured to the wall. 4e) the water temperature registered 124 degrees F. 5a) the outlet shutter near the dining table was stuck in the open position. Jane closed it when licensor pointed it out. It is no longer automatic.
