On 3/26/2026 around 12:30pm, this licensor conducted an unannounced monitor visit to this Provider's program. One of the Provider's staff (name not given when requested) answered the door, but would not allow this licensor inside when this licensor stated who they were, why there were there, and showed their official DCYF identification badge. After the staff called the family home owner, the phone was given to this licensor. The family home owner stated, yes they were open with children in care (licensor observed three children waving to this licensor through the window), but the provider stated they were at the store and requested this licensor do an inspection another day. This licensor stated they could begin the inspection while the provider finished their shopping, but the provider once again stated no, they wanted this licensor return another day. This licensor stated per WAC 110-300-0035, licensees must grant access to department licensor during program hours. The Provider once again stated they wanted this licensor return another day.

At the time of inspection, the outdoor emergency exit gate did not self-close or self-latch. The gate was equipped with both mechanisms, but it could not close or latch without assistance.

At the time of inspection, the right end of the chain link fence was rolled inward, creating a large opening in the fence. A large gap was observed under the non-emergency exit gate that leads to the neighbor's property on the right side of the outdoor space.

This licensor observed wooden planks with rusty nails sticking out of them near the back fence of the outdoor play space. Protruding nails were also observed on the outdoor emergency exit gate.

At the time of inspection, two of the five reviewed child files did not have updated immunization records. These two files did not contain parent or guardian signed and dated statements detailing when the immunization records would be updated.

There was a broken rocking toy (horse) that was laying in the high grass. There was also rocking toys (horses) that were observed on the wooden deck. There was a climbing structure and rocking horse on the cement walk way. The yard at this family home is grass and the cement walk way.

Yearly fire marshal document was not observed and was provided to the provider during this visit to complete. Vent above changing table needs cleaning.

Storage of hazardous and maintenance supplies · WAC 110-300-0260(1)(a) · Serious

There was body wash and shampoo/body wash that was observed in the shower accessible to children. The provider moved the items under the sink which has latches in the door but the latches need to be readjusted to securely latch and catch the top of the cabinet frame.
