At the time of inspection, a small slide was observed within two feet of the back fence. The wood chips under this slide was measured to meet CPSC guidelines.

At the time of inspection, the cabinet underneath the restroom sink was accessible to children. No lock or method to prevent child access was observed. Toothpaste, razors, used toothbrushes, and other restroom items were observed in this cabinet.

Documentation of a food handlers card was not observed in the provider's file. The food handlers card reflected in MERIT is also expired. Provider did not have documentation of current BBP training.

Equipment was thrown all over the backyard and stacked on top of each other. Some items observed to be broken and not with required clearance. It appeared to be a small slide that had broken piece and was thrown on top of the other items in the yard. There was a Corona beer bottle in the yard along with juice box and other trash debris. The licensor pointed out the beer bottle and other trash in the yard from the porch to the provider and she stated it was from the upper level and tossed in the yard. Need to ensure that required ground covering and 6 feet clearance is around climbing equipment.
