During the inspection, Licensor Harris observed a swing and slide set that did not have proper ground covering. Provider will remove swing and slide set from the backyard and will not allow children to use. (1)(b)

Provider will remove from home (2)(c) During the inspection, Licensor Harris observed two shelves in the play area that need to be mounted. (3)(f) During the inspection, Licensor Harris observed a hose in that backyard that prevents a tripping hazard (3)(e) During the inspection, Licensor Harris did not observe a push gate installed at the entrance of the stair opening located at the lower level of the home. (4)(f)(C)(iii) During the inspection, Licensor Harris observed a closet sliding doors in the play room that were not placed on the track properly. Provider will need to fix closet sliding doors (3)(g) During the inspection, Licensor Harris an outlet near a water source (near kitchen sink) that is not GCFI and tamper-resistant or inaccessible to children (5)(b)

Provider needs to cover furance preventing children accessing the units (3)(b)(ii) During the inspection, Licensor Harris did not observe a carbon monoxide detectors located in the licensed area (3)(g)(ii)
