At the time of inspection, this licensor reviewed five child files. All five child files were observed to be missing current, completed immunization records. None of the five reviewed child files contained a parent or guardian signed document detailing when the child's immunizations will be brought up to date.

At the time of inspection, this licensor observed multiple body and hair cleaning products, labeled keep out of reach of children, accessible on the floor of the restroom shower.

Infant safe sleep practices · WAC 110-300-0291(1)(h) · Immediate

The licensor observed in the cribs that in one crib there was pillows and blankets and the other crib there was a blanket draped over the side. During the visit there was an infant present and a 1 year old that occupies the cribs but they were not in the cribs at the time. The licensor discussed with the provider and assistant, S.M to not store items in the crib nor on the side of the crib even when children are not occupying them. They had just done wash day and items had been placed in the cribs and not yet removed. The licensor discussed to ensure that it is not even stored in there. Once washed, fold and put away in the closet/cubbies. Items were removed during the visit by assistant, SM and placed in the cubbies/closet. Later when the infant and toddler went to sleep there was no items in or on the cribs.

Child with food allergy has an epipen but the epipen was not present onsite. According to the provider and assistant, S.M the child had left the program and was gone for 3 months. Care resumed for the child at the program on 6/21/2024. Licensor spoke to the parent during pick up about needing to bring a epipen to the program for them to have onsite for the child.

Children files were missing current CIS/Vaccination records. (2/5 children files). This was cited during the visit conducted on 11/13/2023.

Prescription medication was not present for review. In restroom there was body cream, face cream, vaseline, toothpaste (colgate and crest) and face wash in the mirror cabinet and accesssible to children.

Outdoor early learning program space · WAC 110-300-0145(1) · Short Term

Play area had been changed since last visit and was remeasured. There is a kitchenette and a basketball hoop that required securing. The grass had been recently cut according to assisant, S.M so items had been moved in the yard. Stairs that were built as an exit out of the family home window has an opening at the bottom of the first stair that is 1' 6". Need to add a shaded to the play area. It was getting hot during the inspection of the yard.

Door has a deadbolt and lock on the door knob. The door knob was locked when the licensor knocked on the door and once entered to conduct the visit. To allow access of parents/guardians or even to get out you have to unlock the knob first and then turn the handle. This is a two hand manuever to gain access or exit. Licensor sent the provider an example of a door knob that meets the WAC. Window used as exit from backroom is required to be unlocked during child care hours.

Children files were missing documentation. Licensor discussed with provider and with 1 parent regarding missing information. 1 child does not have a dentist so parent will write a plan of what the providers are to do if child sustains injury to teeth or gums while in care. Licensor showed and highlighted the missing information from 3 of the 5 files.

Table and high chair was observed to be just wiped with a paper towel. No 3 step process. Utensils were observed to not be cleaned via the 3 step process. Banana was cut, knife wiped off and put back into the cabinet. Freezer needs cleaning up wiped/defrosted has thick frost on items in the freezer.

Grass needs to be cut. Basketball hoop is not secured and was observed turned over in the yard along with other play equipment. The rocking horse in the yard is on grass and does not have proper ground covering. When walking down the driveway to the door of the family home childcare there is a gate located by the door and the gate is not locked nor latched. The gate was observed open and there is a large trampoline that is in that yard. Trampoline is accessible to the children because the gate is not closed nor locked.

Window blinds were observed on 4 windows. When pulling the cording on the left it formed loops. This looping was shown to the provider and she was informed she has to be removed them. On some of the windows the provider has blinds and curtains. The licensor suggested that she just use curtains only. Little kitchenette play area can tip. The kitchenette requires securing.
