During the inspection, this licensor observed a damaged Pack 'n Play in the sleep room. The Pack 'n Play was crooked and wobbled when touched. The doorknob on the restroom door was observed by this licensor to be damaged.

At the time of inspection, the outdoor emergency exit gate was observed to be damaged. Due to the damage, the gate was crooked and could not self-close or self-latch. Multiple sections of the fence are cracked and missing paint.

During the inspection, this licensor observed an outdoor playground slide arranged inches from the outdoor fence with the slide leading directly into the fence. This ground under outdoor playground slide did not have any shock absorbing material.

At the time of inspection, the front door emergency exit could not fully open due to being blocked by a couch. The Sleep Room emergency exit window was blocked by a plastic stackable drawer cabinet.

During the inspection, this licensor observed the following in the restroom: The handwashing sink could barely drain water. This led to a large accumulation of dried gunk around the drain. Dirt, stains, and other debris on the floor. Crayon marks, dirt, and stains on the restroom door and walls. Large occupied spiderwebs in the ceiling corners. Cockroach carcasses were observed spilling out of the light fixtures. No 3-step cleaning bottles. No handwashing soap was available. There was soap underneath the sink, but it had not had it's seal broken for use. Damaged floor trim with chipping paint. Large cracks on the walls.

There was no files for the licensor to review. Documents were damages. Licensor provided the provider with a new drill form to document the monthly and quarterly drills starting in August.

Program has a cleaning schedule but appeared to not follow it. There was no garbage can in the restroom. There were no 3 step processes for any of the licensed spaces. There is no table for the children to sit and eat at so food was was being eaten on couch i main space, floor and in the bedroom on a mattress. The children were eating granola bars and cheerios and crumbs were falling on the couch, floor and mattress.

Provider nor the children were observed washing their hands. There was no soap in the restroom and a child announced they needed to go to the restroom. Children ate food, jumped and played on the mattress and other items in the room and got snacks to eat.

There was 3 children that were present for child care and providers 3 children. Files were not found or present for any of the 5 children. 1 child child care child the provider had the immunization record on her phone for the licensor to review

Program gate to the front yard is broken knocked down in areas and no longer self-closing and self-latching. The fences that were on the sides of the house to ensure children did not access the unlicensed spaces is broken and knocked down and/or has large openings and there is unsafe hazardous materials that the children can access on the sides of the house. On one sides of the house the gates that are broken hardware screw/nails are protruding. The front licensed play area had two hoses thrown across the yard and what appeared to be a pressure washer in the front yard by the front door. The cording for the pressure washer is laying across the yard. There is play equipment possibly a hoop that is broken and thrown in the yard. On the porch leading leading into the home there was turtle wax car wash.

Plastic outlet covers were observed in the bedrooms and main space. Licensor removed the outlet covers out of the outlets and informed provider that they were choking hazards. Plastic bags were in the bedroom in the open and accessible closet at child level. The licensor gathered plastic bags and had the provider put them on a high up shelf that was above our head to store the bags. Water hoses and cording for the pressure washer was accessible to children. Wood fences are cracked, knocked down that are on the sides of the homes. There is nails/screw protruding from the broken fences. On the sides of the house there is unsafe materials/equipment that the children have access to since there is no longer a barrier. In unlicensed space there is a washer and dryer. During the visit the children were freely going into the unlicensed space which is also where the kitchen is located. Chipping paint was observed on walls around door frames and baseboards.

Flashlight was no working and there was no current or extra batteries. The location of the couch by the front door does not allow the door to open all the way up. The door hits the couch.

The program has safety plugs protectors in outlets in licensed spaces which pose a choking hazard. TV cording needs securing to the wall in the child care space. Plate for lights in restroom is cracked and needs to be replaced. Bulbs in restroom do not appear to be shatterproof when the licensor tapped on them. Secure kitchenette it was easy to tip.
