The large Background Discovery playground structure failed to meet the six foot safe use zone for the rear ladder and the tube slide. This structure also failed to meet the safe use zone requirements of twice the height of the top bar from which the swing is suspended.

There are lots of plastic bags (grocery bags) stored and accessible to children in the restroom. (cited last monitor visit) Need to secure the plastic kitchenette and vanity are not secure they can easily tip (cited last visit) Baseboard by Swahili numbers missing/chipping (cited last monitor visit) Latch on the dryer is broken and has been secured with clear packing tape. The shoe rack in the kitchen and the the little white table (cited last monitor visit) in the child care space was observed to have chipping paint. Outlet in the restoom was not GFCI nor tamper resistant. Outlet in kitchen does not appear to be GFCI and tamper resistant (cited last monitor visit)

CPR/First Aid observed in provider and assistant files was online and no longer approved. The first aid and CPR training had just been completed in January. Bacitracin, alcohol wipes and other wipes and ointments were found in the first aid kit and need to be removed.

The provider and assistant, CIK is missing documentation of current BBP. The provider, and assistant, CIK did not have documentation of approved CPR/First Aid. Assistant, CIK was missing documentation of current Food Worker card. Assistant, CIK had a certificate for Safe Sleep in her file dated 1/29/2024 but there was nothing reflected in MERIT for the training. the training for Safe Sleep in the MERIT system for CIK was also not verified dated 3/8/2022-3/8/2023. The Safe Sleep Training only reads confirmed only in MERIT and not verified. Licensor informed provider and assistant that they needed to follow up with MERIT.
