(2)(c) - Yellow cords were observed hanging from the gazebo rail outside. (2)(f) - No posters or stickers were observed at children’s eye level on the glass sliding door in the kitchen. (3)(c) - A clear broken plastic piece was observed on the ground outside. (3)(f) - A basketball hoop on a plastic stand was not secured and at risk of falling. (4)(e) - The temperature of the bathroom sink was measured at 139 degrees Fahrenheit. (4)(f) - The outdoor stairways lacked handrails, increasing the risk of falls for children. (4)(f)(C)(iii) - The stairways leading upstairs require a pressure gate for safety. (5)(a) - Outlets throughout the facility were observed without TR covers. (5)(b) - The outlet near the bathroom sink was equipped with a GFCI button but was missing a TR cover.
