At the time of inspection, the provider's outdoor play structure did not meet CPSC guidelines regarding safety fall zones and surfacing. The safety fall zone and surfacing at the bottom of the double slide measured 4.5 feet with 1-2 inches of wood chips instead of the required 6 feet with 9 inches of wood chips.
