1 child file was missing the 2nd page of the medication form for Zyrtec/Cetirizine. The child also did not have this medication onsite. Need to ensure that medication forms are reflective for application amount on the diaper forms. One child record for diaper ointment was observed to have an end date in January 2024 for diaper ointment but it was still being applied.

Child has food allergies but does not have an Individual Plan signed by the parent/guardian and the doctor. Child has asthma but the medication needed was expired as of October 2022

A child had albuterol (expired) that was not in a box and did not have a plan that would specify the required information for administration, side effects, and other information specified in the WAC from the doctor. Forms for medication were not filled out in accordance of WAC and Health Care Plan for diaper ointments and sunscreen. There was a child that had Tylenol for teething and read as needed. The meds had been at the program for several months (11/21/22) and there was no end date for usage. Need to ensure that all areas of the forms are filled out. Also need to ensure that labels are followed. If the request is outside of the the label reads for usage then this would require approval from a doctor. Medications/oitments/diaper creams/sunscreens were not labeled with first and last names.

Cording from equipment was loosed and needed to be secured to walls or shelvings to reduce the risk of strangulation throughout site. No loose cording and or dangling cording should be present and accessible to children. Cabinet in Pre-K room with plastic bags needs a latch. Secure shelving unit in the Pre-K room and heavy items throughout that are stored on shelves that can potentially fall during an earthquake.

Staff were missing current CPR/First Aid. The site will be conducting training for staff to complete their CPR/First Aid in a few weeks. Staff did not have current BBP training. BBP is required yearly. Staff were missing documentation of Safe Sleep Training and current Food Handler's. Information must be reflected in MERIT
