The outdoor gate that is used as an emergency exit does not self-latch when released from an individual's control. 110-300-0145 (3) There were no shaded areas observed in the outdoor play space. 110-300-0145(4) There were no materials observed to promote physical activity for school age such as jumping , skipping and pulling.

Stickers on the sliding glass door, are peeling and pose a choking hazard. Blue painters masking tape on a post in the main play room, poses a choking hazard. Blinds in the dining room area where the children's cubbies are, have not been retrofitted and form a loop posing a risk to children. Plastic bags on sleeping mats are accessible to children on a shelf. Mats that have no sheets on them. Metal post in large room off the dining room, has blue masking tape around it, is sharp and still accessible to children. Poses a cutting hazard. Sliding closet type doors in the main play room off the dining room, are not secured and pose a pinching hazard to children. Closets need locks to secure them from children accessibility. Kitchen that is accessible to children with no gate blocking it, needs locks on refrigerator and pantry to prevent entrapment. Fake grass covering on deck and in the backyard area, are not secured and post a tripping hazard. Floating shelves over children's heads, have plastic containers with files in them. They are unsecured and when this licensor touched one lightly, it fell to the floor breaking the container. Objects pose a falling hazard to children. Cubby for children needs to be secured to the wall to prevent tipping over hazard to child Outlets in the dining room and living room, are not trr. The outlets in the room off of the dining room are the twisting kind and not trr. Gas stove in the kitchen is accessible to children as no gate was observed. The stove top does not have a guard around it and the front knobs accessible to children, need child safety caps.

Has not completed a fire marshal inspection on the state fire marshal form or had a government agency inspect the home for fire safety issues. The provider does not have documentation that states when the fire extinguishers were maintained or purchase. The fire extinguisher in the room off the dining room is not 2 A or above, but a smaller and thinner fire extinguisher. Smoke Detector/carbon monoxide detector is mounted to the wall and not the ceiling in only the room off the dining room. Needs to be mounted on the ceiling.

WAC 110-300-0165(4)(e): Water temp in the licensed bathroom is 129 degrees, limit is 120.
