At the time of this inspection, provider L.C. and assistant teacher J.A. did not have current documentation of BBP training. BBP training for both staff members expired as of 6/30/2024 At the time of this inspection, an infant was present in care and observed taking a nap while being supervised by assistant teacher J.A. Infant safe sleep trainings for L.C. and J.A. were expired as of 6/27/2024.

At the time of this inspection, provider did not have documentation of required vaccinations for 2 pet cats.

At the time of this inspection, licensor observed mouth wash labeled "keep out of reach of children" stored in a bathroom drawer accessible to children. Violation corrected on site. Licensor observed a toilet brush in a storage cabinet under the bathroom sink that was accessible to children. TA provided to put a child safety lock on this cabinet to make it inaccessible, or to store the toilet brush in another location inaccessible to children.

The restroom cabinet and items in the baskets in the restroom on a shelf are considered medications (creams, hair products, toothpaste etc.) that belong to the household members. These items were accessible to the children.

There was no ground covering under the little slide in the front yard. There were scooters and bikes with missing grip handles and metal exposed.

There is yard equipment play houses with chunks of wood gone splintery to touch. Heater on wall in room by back door (TV Room) was observed to have a heater that was hot to touch when turned on. The heater requires a guard around it. Dryer requires a lock. The washer has an automatic lock that enables ability to open. Items stored on shelves throughout that can become projectile objects need securing. Wii games, books etc. There are some shelving units that are on carpet and are wobbly to touch not secure Powe strip in other Bedroom with Bay Window was observed to have a powerstrip sitting on one of the shelving units accessible to children (several items plugged in at time of visit). Also had a collection of dust on it as well . White shelf in other Bedroom with Bay Window needs securing Light Bulbs were observed to not be shatterproof. Main Space toys on higher shelves need securing. Razors in restroom in a basket was accessible to children. Play kitchen and Little Tikes Grill Set was thrown on playground was not secure unstable.

Disinfectant/Bleach is stored in the cabinet in the restroom with no lock on the cabinet and is accessible to children. Cleaning products, other hygiene and beauty items are stored in the cabinet in the restroom and in baskets in restroom accessible to children. Janitorial items by the backdoor outside was moved during the visit.

Assistant Teacher/Household Member is missing required trainings and has unsupervised access with children. All staff are missing BBP training. Assistant Teacher/Household Member is missing CPR/First Aid, Food Handler's Permit, Initial Basic STARS, DCYF Trainings: Recognizing and Reporting Abuse, Emergency Prepardness Training, Shaken Baby Syndrome, Families Experiencing Homelessness, Safe Sleep and Medication Management. Owner/Provider missing DCYF Trainings: Emergency Prepardness Training, Shaken Baby Syndrome, Families Experiencing Homelessness, and Medication Management.
