Retrofits are allowing for blind cords to form loops posing a strangulation risk to children. Retrofits do not cover the whole blind cord posing risks to children. Retrofits are a bracket that the provider wraps the cords around and 1 blind was not secured and formed a loop. Red yarn strings noted on the climber that would be able to form a loop around a child's neck and poses a strangulation risk. Plastic bags observed in diaper changing bathroom accessible to children under the sink in a container. Plastic bags pose a safety risk to children. Cabinets under sinks have no locks, are empty and pose an entrapment and hiding hazard for children.

Cleansers and disinfectants are kept accessible to children in the diaper changing station. Aerosol in a spray can and one plugged into an outlet observed in the bathroom where children are changed. Several paint cans and one ice melt accessible to children in an outside unlocked shed. Shed is not locked making the contents accessible to children. Viewed in the shed included bag of ice melt, cans of paint, ladders and other maintenance type tools. All accessible to children as the door was unlocked.
