In one room, only one blind cord is no longer retrofitted with the tube missing. The cord forms a loop posing a strangulation hazard. Cords are viewed accessible to children in the toddler room. In 2nd room, blinds are not retrofitted, no tubes present, all cords form a loop that poses a risk of strangulation to children. Cords are viewed accessible to preschool age children.

WAC 110-300-0186(4) is valid. During the interview chef Cassie said that she was the one who assembled the sandwiches for the children, Cassie admitted to adding cheese to the child's sandwich. Cassie said she was trained on the child's individual care plan and that there is a chart posted in the kitchen which details the children's food allergies. Cassie did not review the child's individual care plan prior to serving him food.
