At the time of the onsite monitor visit, there was a plastic tan climber platform with a green slide that did not have a 6' safe fall zone extending in all directions. A concrete wall was less than 3' from the climber toy. At the time of the onsite monitor visit, the wood chips unnder the tan climber platform with a green slide was less than an inch deep.
