The diaper changing area did not have a designated diaper disposal container that was hands-free, lined, or within arm's length of diaper changing area.

The appliances used to prepare food had used pots and utensils and food debris and were not cleaned after each use. The diaper changing table did not have cleaning and sanitizing solution nearly to be used after each use. Play pen side and couch had visible stains.
