The assistant was missing their file that included their first and last name, date of birth, job title, first and last day of employment, and proof of professional credentials. 110-300-0115(3) Licensor reviewed staff files and observed them not to be complete. The assistant was missing their file that included current government issued photo identification, emergency contact information, and a completed employment application or resume.
