At two points during this inspection, a provider was left alone with eight children, three younger than two years of age outside. One provider stated that she normally takes children inside to change diapers as needed, leaving her peer outside with the remaining children. The licensee states he rarely changes diapers.

(2)(b) Window blinds were in place at both windows in the child care. One window blind was easily removed and removed immediately. (2)(c) A cord used to plug in a table top water fountain was accessiblet to children in care. This cord was made inaccessible immediately. (2)(d) Plastic gloves and bags were accessible to all children in drawers in the bathroom. (4)(d) The bathrooms , living, and dining rooms tested at 60 degrees upon my arrival. The family room tested at 63. This is where the heat pump is located that the provider was hopeful upon licensure would heat all licensed spaces. It does not appear to be working. By the time of departure not all rooms could be 68 or higher at all times.

Consent to seek care and provide treatment did not reference the licensee on some children's files. Date of last physical and dental exam was not on several children's files. Dentist or alternative medical facility was not listed for some children.

The window in the living room, which is the primary fire exit was blocked by a stick. It also had a curtain which affixed to the window with suction cups making it difficult to open unless one removed the curtain entirely.

The applicant has put outlet covers on outlets that are actually choking hazards. There are exposed dangling cords in the infant room and carport area. The applicant has not quite figured out how to keep the infant warm enough in Winter. There is a small cadet heater in the infant room but it is accessible to children so cannot be used. It will be hot to the touch. None of the outlets accessible to children are TR type.
