At the time of the onsite inspection visit, there was a blue and orange plastic slide that was 6" from the concrete patio and less than 6' from the back fence. At the time of the onsite inspection visit, there was an orange and blue slide that did not have a safe fall zone material that complies with CPSC guidelines underneath. At the time of the onsite inspection visit, there were blinds with internal strings that could form a loop that was at least 7" and accessible to children in the living room and bathroom. At the time of the onsite inspection visit, the TV wire was three feet long, hanging loose, capable of forming a loop around a child's head, and accessible to children present. At the time of the onsite inspection visit, the indoor environment temperature was measured to be 64 degrees Fahrenhiet, with the windows open (60 degrees outside), and the heat turned off while a child was present. At the time of the onsite inspection visit, there werer 5 outlets observed that were not tamper resistant and accessible to children. (3 in dinning room and 2 in living room)

At the time of the onsite inspection visit, the prefered medical and/or prefered dental providers section was left blank on five of the child files observed. At the time of the onsite inspection visit, the last physical and/or last dental exam date was missing from the five child files observed. At the time of the onsite inspection visit, the consent to seek medical care and treatment section of the child registration form was missing a signature from the parent in four of the child files observed.
