This is a direct violation of stipulation (L) on the probationary license. Probationary stipulation (L) states "Hazardous items, including items labeled “keep out of reach of children,” must be inaccessible to children at all times, both indoors and outside. Choking hazards must be inaccessible to children under three (3) years of age".

Staff stepped out to get the name to face from the classroom and corrected the name to face sheet while specialist was present in the gym.
